John Collins ‘Sonny’ Polk, DMD, 81, Lebanon
John Collins “Sonny” Polk, DMD, 81, of Lebanon, died Sunday, Dec. 28, 2014, at Audubon Hospital in Louisville.
He was born in Lebanon and graduated from Lebanon High School. He began his college studies at Western Kentucky University where after three years he received early admission to University of Louisville Dental School. Upon graduation he served four years as Captain in the U.S. Army Dental Corps. He returned home to Lebanon where he continued his dental practice for 54 years.
He was a member of Lebanon Baptist Church where he taught Sunday School and served as a deacon and Sunday school director for years. He served on the Board of Overseers at the University of Louisville, where at the end of his term he was named emeritus. He was an avid U of L fan and will be missed by many.
He was preceded in death by his grandparents, Luther and Georgia Collins; his mother, Mary Elizabeth Collins Polk; one sister, Margaret Polk Glasscock; one brother-in-law, Bill Glasscock; and one grandson, John Dulmage.
He is survived by his wife, Caroline Chelf Polk, two children, John Collins Polk, Jr. and Leslie Polk Dulmage; one sister, Georgia Polk (Jim) Childers of San Antonio, Texas; one grandchild, Kurt Dulmage; and several nieces and nephews.
The funeral is 2 p.m. Wednesday, Dec. 31, 2014, at the Lebanon Baptist Church officiated by Dr. David Whitlock, Dr. Wesley Noss and Dr. Kevin Lee. Burial is in the Ryder Cemetery.
Visitation is 3-8 p.m. Tuesday, Dec. 30, 2014, and 8 a.m.-2 p.m. Wednesday, Dec. 31, 2014, at Lebanon Baptist Church.
The Bosley Funeral Home in Lebanon is in charge of arrangements.
